	The clinic begins September 22 – when members are invited to stop by and
	meet the program’s dietitian - Linda Davenport, RD. Cost for an initial
	consultation and a follow-up session is $35.&lt;/span&gt;

	&lt;b&gt;What happened?&lt;/b&gt;
&lt;/h3&gt;

&lt;p&gt;
	Carney Hospital has become aware of a potential breach of patient
	confidentiality by an external, independent billing agency that
	contracts with an independent pathology group that works with Carney
	Hospital.&amp;#160; The billing agency disposed of patient billing
	information by sending it to a landfill, rather than destroying the
	information in accordance with industry standards.
&lt;/p&gt;

&lt;p&gt;
	We are currently investigating this matter.&amp;#160; The security of our
	patients’ personal information is of the utmost importance to us.&amp;#160; Our
	understanding is that the billing records that were disposed of in the
	landfill have been destroyed.&amp;#160; We believe the records were sent to
	the landfill during a purge of old billing records at the billing
	company.&amp;#160; At this time, we believe that the billing company did not
	regularly dispose of their records in this landfill and that this is an
	isolated incident.
&lt;/p&gt;

&lt;h3&gt;
	&lt;b&gt;Whose information was involved?&amp;#160;&lt;/b&gt;
&lt;/h3&gt;

&lt;p&gt;
	Our current understanding is that the potential breach is limited to
	pathology patients who had tissue studies conducted from 2007 though
	2009.&amp;#160; All impacted patients whose billing records were involved
	will be notified.
&lt;/p&gt;

&lt;h3&gt;
	&lt;b&gt;What types of information were on the documents?&amp;#160;&lt;/b&gt;
&lt;/h3&gt;

&lt;p&gt;
	The information on the documents includes names, addresses, dates of
	birth, Social Security numbers, insurance information and in some cases
	diagnoses relating to the pathology tests.&amp;#160; The documents contained
	no financial or credit card information.
&lt;/p&gt;

&lt;h3&gt;
	&lt;b&gt;When did Carney Hospital find out about this incident?&lt;/b&gt;
&lt;/h3&gt;

&lt;p&gt;
	We learned about this situation on August 5, 2010 and immediately
	launched an investigation.
&lt;/p&gt;

&lt;h3&gt;
	&lt;b&gt;How can I find out if my records were improperly disposed of?&lt;/b&gt;
&lt;/h3&gt;

&lt;p&gt;
	All impacted patients whose records were involved will be notified.&amp;#160;
	We believe the responsibility for this notification belongs to the
	billing agency and the independent pathology group.&amp;#160; Carney
	Hospital will work with them to ensure that all impacted patients are
	notified and appropriate follow-up action is taken.
&lt;/p&gt;

	&lt;b&gt;Were records from the other hospitals in the Caritas system involved
	in this incident?&lt;/b&gt;
&lt;/h3&gt;

&lt;p&gt;
	No.&amp;#160; The independent pathology group and the billing agency did not
	work with any other Caritas hospitals.
